  1. Print & Fill out a Kidney Kars Donation Form (donation form below)
    2. Mail in Donation Form

  2. Correctly sign your title over to the NKFU & I
    (click to view detailed instructions on how to transfer your title)
  3. Mail the Kidney Kars Donation Form & title signed over to:
      NKFU
      3707 North Canyon Road, Suite 1D
      Provo, UT 84604
  4. Towing company will call you in 3-5 days after title is received by the Foundation.
